Michigan Visiting Care's Geriatric Care Management Program is here to help.

Many Michigan seniors are faced with great difficulty in meeting their care needs on their own. Family members are often challenged when trying to care for aging parent(s) and loved ones. Michigan Visiting Care's Care Management Program is an excellent choice for providing assistance to individuals and families who need additional help.

Our program is a collaborative effort between a trained and certified care manager and social worker, and a registered nurse. Our care manager provides assessments, care coordination, advocacy, and other related case management activities as needed. A thorough assessment of the client's requirements is of paramount importance. To insure accuracy and completeness, we prepare each client's assessment collaboratively with family, physicians, and ancillary health care providers. Our goal is to ensure quality and cost-effective care for our clients. As part of the University of Michigan Health System and Home Care Services, our program can also provide easy access and navigation to the many services available through the University of Michigan Health System.
