HouseCalls
The Michigan Medicine HouseCalls program is an interdisciplinary team of nurse practitioners, a care manager, and a pharmacist who provide care to patients that are homebound or have limited mobility. In coordination with the HouseCalls medical director and/or the patient’s primary care provider (PCP), the HouseCalls team provides care over time through care management visits, and regular care through transitions of care visits, one-time assessments and urgent visits.
The HouseCalls team services patients that live in Washtenaw County and Belleville, and operates from 8-5pm Monday-Friday. After office hours and on weekends, the HouseCalls medical director responds to the patient. Patients must be referred to the HouseCalls program by a Michigan Medicine provider.
