Mental Health Services
The Michigan Visiting Nurses Mental Health Program can assist patients with psychiatric disorders to live in a safe and supportive environment.
Our in-home services include:
- Home environment and patient safety evaluations
- Reintegration of a psychiatric patient discharged into the home environment
- Coordination of care with the psychiatrist, primary care physician, specialist physicians, community agencies, family and patient
- Assessment and monitoring of psychiatric symptoms and current stressors
- Medication assessment and education
- Supportive counseling to the patient, caregivers and family
- Education of disease processes, symptom management, and relapse prevention
- Assist families and patients coping with cognitive and behavioral changes associated with dementia
- Assist individuals who experience anxiety and depression related to a medical illness(es)
- Support and problem-solving with patients and families facing end of life issues
