Staff Bios
Grace Jenq, MD: Medical Director
Dr. Grace Jenq received her medical degree from Johns Hopkins University. She completed her residency and chief residency in internal medicine at University of Alabama Birmingham. She continued her fellowship training in Geriatrics at Yale University and stayed on faculty from 2005-2016. At Yale-New Haven Hospital she served as the Associate Chair for Patient Safety and Quality and the Medical Director for Inpatient Medicine. In 2016, she joined the University of Michigan Division of Geriatrics and Palliative Care Medicine. She is now a Professor in the Division of Geriatrics and Palliative Care Medicine at University of Michigan and the Associate Chief Clinical Officer for Post-Acute Care at Michigan Medicine. She cares for patients in the Michigan House Calls Program, Patient Monitoring at Home Program and the newly established Hospital Care at Home Program. Her quality improvement and academic interests lie in how to create better models to care for elderly persons within their own homes and how to enhance transitions of care out of the hospital by engaging clinicians and patients/caregivers and improving system design. In the Department of Medicine, she is a member of the Clinical Excellence Society and she received the Chairperson’s IMPACT Award in 2021.
Jan Doolittle, MSN, BSN, CNML
Jan Doolittle is the Associate Chief Nursing Officer for Primary Care and Post-Acute Care Services at the University of Michigan Health System. She has 38 years of experience across primary and specialty care having held various nursing leadership positions in the inpatient, ambulatory care and community arenas. Throughout her career, Jan has led and implemented several successful programs to address unacceptably high readmission rates. She has previously presented at the American Academy of Ambulatory Care Nursing National Conference on an innovative and novel care model supporting transitional care coordination.
Jan has a passion for population health and believes outreach and intervention through evidenced based practice are paramount in supporting patients in self managing their chronic disease.
Jan holds a BSN from Madonna University in Livonia Michigan, a MSN from the University of Detroit Mercy and a CNML certification through AONE. Jan is an active member of several professional organizations and is adjunct faculty at the University of Michigan School of Nursing.
Maryann Riggs, PharmD
Maryann is the Nursing Supervisor and Clinical Manager for the House Calls, Patient Monitoring at Home and Subacute Care Programs. She has 30 years of Nursing Experience in various clinical settings such as Intensive Care, Post Anesthesia, Urology, Radiology, Cardiology and Care at Home. Maryann is a graduate of Eastern Michigan University where she has earned her B.S.N and M.H.A. Maryann is very passionate about providing high quality / patient centered care, with attention to measures to reduce re-hospitalization and provision of care which enhance quality of life.
Mary Ann Nicholas
Mary Ann is an Adult Nurse Practitioner. She received her Bachelors of Science degree in Nursing from the University of Michigan in 1987. She practiced as a registered nurse in critical care, the emergency department and procedure areas both at University of Michigan and at St Mary Mercy in Livonia. In 2012 she received her Masters of Science in Nursing with a Post-Graduate Adult Primary Care Nurse Practitioner Certification. She practiced for a year in a Sub-Acute Rehab setting then returned to the University of Michigan in 2013 to work inpatient on the Advanced Heart Failure and Transplant Service. From 2015-2020 she worked inpatient and outpatient for the department of Family Medicine. In 2021 she joined the Post Acute Care Services Provider team and is now the lead nurse practitioner for the Patient Monitoring at Home Program.
Stacey Bajema
Stacey is a care manager with the HouseCalls Program. She joined the HouseCalls team in October of 2019. She earned her bachelors of social work from Eastern Michigan University in 2008, and came from Community Mental Health where she worked with patients with severe mental illnesses and co-occurring disorders. As a part of the HouseCallls Team, she provides community based medical social work and care management to complex, vulnerable homebound individuals. In her free time, she enjoys spending time with her family, eating at new restaurants and being outside.
