About Us
A medical condition can strike when least expected, causing a great deal of stress on everyone involved. To make recovery as comfortable and quick as possible, we begin the home care plan before the patient leaves the hospital or clinic. Our experts work with the medical team to develop a plan of care, so the transition to home will be smooth. At the patient’s home, our health care professionals will train the patient and family to manage care, making sure everyone is completely comfortable with the care plan.
Making a Difference
Patients, their families, visitors and staff can recognize employee contributions through our Making a Difference award. Just go online or use one of the paper nomination forms available in clinics or departments. Awardees receive a pin, a Making a Difference certificate and are invited to a special awards reception.

If you have a concern, the recommended first course of action is talking to a member of the health care team that provided your care, treatment or service. You can contact your Michigan Visiting Nurses health care team using our toll-free number, 1.800.842.5504.
If a health care team member is unable to resolve your concern, please contact Patient Relations & Clinical Risk. Our staff provide an objective environment for you to raise concerns and ask questions. We will address your concerns in a fair and reasonable way, and ensure an impartial review.
