Hospital Care at Home

The Hospital Care at Home program is a care concept that aims to reduce inpatient hospital care delivery and instead manage select acute medical diagnoses (Congestive Heart Failure, Cellulitis, Chronic Obstructive Pulmonary Disease, Pneumonia, Urinary Tract Infection) safely in the comfort of the patient’s home. This care model has been successfully implemented and effective in the U.S. and other countries for a number of years, and now we are excited to partner with BCBS and lead this innovative care delivery program in Michigan.
